Is surgical intervention really required before IVF?

It is matter of great debate whether a laparoscopic intervention is really required before starting an IVF treatment. Some clinicians have made it a protocol to conduct laparoscopy in each and every patient which seeks IVF treatment . Following is my honest opinion on surgery before IVF treatment: 1) According to me, Ovarian drilling 'kills fertility'. So it an honest request that please do not proceed with it unless that is the last resort. 2) Fibroids less than 4 cm in size doesn't require surgery before IVF treatment 3) Less than 4 cm endometrioma(chocolate cyst) doesn't require surgery. Why Multiple IUI is not the suggested treatment for infertility ?

Actually IUI is indicated now a days only during following conditions:  Less than 35 years of the age and AMH is more than 2. Polycystic ovary with precise hormone therapy, meaning no over response or no under response for maximum up to 3 to 4 cycles. Donor IUI in 0 count where IVF is not possible or not affordable.
