When and where to start the IVF treatment?

Patients are confused about the time to start the IVF treatment. Should the age be considered as the parameter or the duration of the treatment? In Indian female, age beyond 35, IVF will be a better option.  More than 5 years of infertility period again strongly indicates IVF treatment . At any point, if AMH is less than 2.0, IVF is strongly indicated. In the era of IVF treatments, 3 IUI attempts are enough. Due to lack of regulations, patients are confused where to start the IVF treatment. There are no set parameters to indicate which is the best centre for IVF treatments. Indian women is 5 year older in terms of fertility power compared to Caucasian population. • After the age of 30, Indian women should be prompt in checking the fertility power through blood test like AMH if they deciding for late family planning. What to do IUI or IVF?

IUI is a low success treatment which usually kills precious time, fertility power of the body,money and leads to psychological distress to the couple. So couples who have time on their hands, more fertility power and psychologically not ready could be offered IUI treatment to start with in the following case: • Young age < 30 years • PCO( AMH > 2.5) • But stop IUI beyond 3 attempts because it doesn't increase the success rates. • By killing fertility power through IUI, it decreases chances of pregnancy through IVF treatment as well. • Gynaecologists can refrain from overcommitting IUI results which will eventually benefits the patients through IVF. Don't go for IUI if: • More than 35 years age • AMH less than 2 • Husband semen count is less than 10 million • Both tubes are bad with grade 3-4 endometriosis.
