Why is Male Infertility increasing drastically as compare to Female Infertility?

According to the the literature, main reason is lifestyle which includes late work, more hour work in a day & Target which Male infertility increasing drastically as compare to female in today’s era. Main reason is lifestyle which includes late working hours, erratic food habits, stress, etc. Pollution and pesticide in food highly affect the sperm production. Radiation emitted by gazettes like mobiles and laptops are extremely harmful for fertility health as well. All of the above factors affect the production  and therefore ejaculated semen sample will have less count & motility(morphology). This disturbs or indirectly damage the DNA in the sperm nucleus called fragmentation of DNA which can be identified by DNA Fragmentation index test. Technique called MACS, basically separates old aged sperms called apoptic sperm to an adherent column of MACS and you will get young and least DNA fragmented. IMSI stands as the gold standard for sperm se...

What are the latest trends in infertility management?

The most important and critical advancement is “only day 5 embryo transfer with freeze more strategy” • As at WINGS, we promote own egg pregnancy in any difficult situation through latest trends in infertility. As mentioned earlier, embryo freezing in advance maternal age (>35y) is the answer. We have to decide clinically whether we do it with or without PGT which is another current advancement option to increase the results. • Currently, egg freezing has also passed the experimental stage. Egg freezing is a good option specially in women detected with any cancer and also for young unmarried women with endometriosis and women who want to plan pregnancy at advance age. • Ovarian Tissue cryo preservation will be also possible in the near future. • Before going for surrogacy option, platelets rich plasma(PRP) and stem cell therapy are possible treatments in very thin uterine lining with multiple ivf failures. • We offer a Kid's embryo scoring software that h...

What is personalized approach in IVF?

Let me try and put this entire concept in a layman terminology. The ability to grow a healthy plant requires a good seed be planted in a fertile soil. Then it requires proper nurturing till it thrives. Similarly, successful IVF requires the same relationship. A good seed is a competent, chromosomally normal embryo and fertile soil is a receptive uterine lining. Step by step approach in this concept can be as under : • It is important to define the variables that affect embryo quality and uterine receptivity for each patient • To avoid "one size fits all" protocols for controlled ovarian stimulation. Instead it is necessary to individualize the protocol to fit the profile of each individual so as to establish an optimally nurturing environment for developing follicles, eggs and uterine lining. • To precisely time the egg retrieval (ER) and then safely extract, eggs from the woman's ovary. • Provide the environment for the embryo to grow till the 5th day...

When and where to start the IVF treatment?

Patients are confused about the time to start the IVF treatment. Should the age be considered as the parameter or the duration of the treatment? In Indian female, age beyond 35, IVF will be a better option.  More than 5 years of infertility period again strongly indicates IVF treatment . At any point, if AMH is less than 2.0, IVF is strongly indicated. In the era of IVF treatments, 3 IUI attempts are enough. Due to lack of regulations, patients are confused where to start the IVF treatment. There are no set parameters to indicate which is the best centre for IVF treatments. But according to me, the following criteria is important in a IVF centre: • More than 5 years of specialty infertility experience of the doctor. • Most latest technology with 0.3 micron clean air modular lab. • In-house and more than 5 years experience of embryology team. • Atleast one triple gas bench incubator. • Day 5 embryo transfer & freeze more strategy for all. Advance maternal age and infertility - wings Hospitals

what is it and what to do? Indian women is 5 year older in terms of fertility power compared to Caucasian population. • After the age of 30, Indian women should be prompt in checking the fertility power through blood test like AMH if they deciding for late family planning. AMH is a screening test by which couples can decide whether they should wait any longer or not. • After the age of 35, it is my simple advise- not to wait more than six months if you are not able to   conceive. • Don't waste your time after age of 35 for IUI, even if financially and psychologically the couple is   not ready for IVF . IUI should be restricted to 2 or less post 35 years of age. • At WINGS, we believe in high end & precise treatment for advance maternal age(>35)  in IVF. We strictly adhere to Only Day 5 embryo transfer, Freeze all, PGS and ERA which are advanced    genetic tests by which we can offer single and healthy pregnancy. Transferri...

What to do IUI or IVF?

IUI is a low success treatment which usually kills precious time, fertility power of the body, money and leads to psychological distress to the couple. So couples who have time on their hands, more fertility power and psychologically not ready could be offered IUI treatment to start with in the following case: • Young age < 30 years • PCO( AMH > 2.5) • But stop IUI beyond 3 attempts because it doesn't increase the success rates. • By killing fertility power through IUI, it decreases chances of pregnancy through IVF treatment as well. • Gynaecologists can refrain from overcommitting IUI results which will eventually benefits the patients through IVF. Don't go for IUI if: • More than 35 years age • AMH less than 2 • Husband semen count is less than 10 million • Both tubes are bad with grade 3-4 endometriosis. So the question that arises is that when to and where to start IVF?

PCO: Unique fertility treatments gives hope to all couples.

India is considered as a hub for PCOD, every zone of India has 12-16% of incidence. Every 6th Infertile couple has PCOD. Therefore a question of worry and thus we need to discuss is it really tough to treat PCO with infertility? • In PCOS , IUI gives 15 -25 % of cumulative pregnancy rate after 6 cycles. But it should be noted that in PCOS, ovary is very sensitive to stimulation and IUI stimulation is never in control by any hormone. So apart from results, multiple pregnancy could not be controlled by IUI treatment , where ovarian hyperstimulation is one of the threat behind the treatment. • After some inventions of injections, hyper-stimulations can be prevented by upto 98% and freeze all embryos strategy will give 100% safety in IVF. • So it can be concluded that in this era of infertility treatment , PCO cases can be successfully treated with robust clinical strategy and experienced embryology team. Infertility: when to meet a Fertility Doctor?

I recommend that you probably don't need to see a  fertility doctor about infertility issues unless you have been trying regularly to conceive for at least one year. However see your doctor, If you are a Woman and: • You are aged 35 to 40 and have been trying to conceive for six months or longer • You are over the age of 40 • You menstruate irregularly or not at all • Your periods are very painful • You've been diagnosed with endometriosis or pelvic inflam-matory disease • You've had multiple miscarriages • You've undergone treatment for cancer  When should a male patient see a doctor? • You have low sperm count or other problems with sperm • You have a history of testicular, prostate or sexual problems • You've undergone treatment for cancer • You have testicles that are small in size or swelling  in the scrotum known as a varicocele • You have others in your family with infertility  problems

Are repeated attempts a reality in IVF? Can it be avoided using the latest techniques?

YES! Usage of good quality injections assist in producing good quality of eggs which ever lead to better embryo formation. Also, high-end techniques like IMSI are required to set qualitative sperm especially in cases with relation to the male factor. But all of the abc techniques require the doctors and embryology team to have more than 5 years of experience Usually all Day 5 embryos will not lead to 9 months pregnancy. So we need to ensure to the best grade embryo is transferred to increase the chances. Gardener's grading system now a benchmark to find out the most competitive embryo. It is important to limit the San cycle transfer only in those couples who have more than 4 Day 5 embryos as success rate  is not more than 20-25%. The remaining embryos can be frozen. But it is of prime important' to note that the concept of vitrification/freezing requires a lot of meticulousness & perfection from the embryology team. Choosing of the normal uterus with 8-9 mm Endonietr...

IVF Success Rate seems to be increasing rapidly.

Today, infertility is a common factor, according to experts; India has witnessed a 20 to 30% increase in infertility rates, in the last few years. IVF process is to treat couples with infertility problems. The result of the IVF treatment is not the same for all, every result is different. Let us look into the factors that will help ascertain the success rate. The first factor is Age. Age plays an important role in influencing the IVF success rate. Younger women between the ages 24 to 34 can conceive because the fertility level is at its peak, hence, more the chances. The IVF success rates under 35 is much higher, well as, the IVF success rates over 40, begins to drop and continues to reduce quickly. At 45, the success rate is at its lowest. Age also affects the quality and quantity of the eggs. This explains the IVF success rate by age and number of embryos. Types of fertility Either one spouse or both can have fertility problems. Women suffering with endometriosis or tube b...

IVF an ideal Treatment for infertile couples seeking Parenthood

An IVF stands for "in vitro fertilization" , in this process the egg is fertilized outside the body or the womb, and then the embryo is re- implanted in the uterus, once the fertilization happens or takes place. Let’s understand the need for IVF? Fallopian tube damage Infertility in Men Infertility in men could be because of various reasons, like, illness, injury to reproductive anatomy, hormone disorders, obstruction, etc. These reasons can affect the sperm count and also prevent conception, either temporarily or permanently. Endometriosis This is a severe disease, women who can’t conceive because of endometriosis, can have a child through IVF. Endometriosis can be treated with the help of medical therapy and surgery. Don’t worry; women suffering with endometriosis, can accomplish their dream, of becoming a mother and experiencing motherhood Infertility related to age If you look at the reproduction life of a woman, the functioning of ov...

Is surgical intervention really required before IVF?

It is matter of great debate whether a laparoscopic intervention is really required before starting an IVF treatment. Some clinicians have made it a protocol to conduct laparoscopy in each and every patient which seeks IVF treatment . Following is my honest opinion on surgery before IVF treatment: 1) According to me, Ovarian drilling 'kills fertility'. So it an honest request that please do not proceed with it unless that is the last resort. 2) Fibroids less than 4 cm in size doesn't require surgery before IVF treatment 3) Less than 4 cm endometrioma(chocolate cyst) doesn't require surgery. Only in cases of extreme pain, surgery should be considered. 4) Pre-IVF Hysteroscopy for all it is totally commercialization from some doctors 5) Adeno myomectomy is also unindicated surgery. Only due to some commercial clinicians, surgery has become a norm for infertility treatment. It is absolutely not mandatory unless doctors are unable to analyse the cause for infert...

Recurrent IVF failures- 'A burning issue of society'

Indian medical fraternity lacks guidelines for IVF regarding the type & standard of lab and instrumentation required. Also, no specific education and qualifications are fixed for an embryologist and the biggest menace is free lancing of embryology which directly impacts the results negatively and thereby increases IVF failures in the society. Main issue in the recurrent failure cases is female egg quantity and quality as multiple attempts depletes the ovarian reserve thereby leading to more failures and thereby needing third party donation. So a structured method adopted from day one of IVF treatment can guarantee a lesser need for third party donation. To improve the IVF results and reduce recurrent failures couples should keep a track of the below: (1) Visit only experienced IVF specialist which cannot be a routine Gynecologist. (2) Need to choose state of the art and advanced IVF lab with high tech instruments with proper QA/QC in the lab (3) In house experienced embryo...

Why Multiple IUI is not the suggested treatment for infertility ?

Actually IUI is indicated now a days only during following conditions:  Less than 35 years of the age and AMH is more than 2. Polycystic ovary with precise hormone therapy, meaning no over response or no under response for maximum up to 3 to 4 cycles. Donor IUI in 0 count where IVF is not possible or not affordable. Erectile dysfunction Unexplained infertility where duration of infertility is more than 2y and less than 5 with age less than 35 and AMH is more than 2 Male sub-fertility when count is more than 10 mill. And motility more than 10 % and morphology is more than 4% and DNA fragmentation index is less than 50 percent Carry home message here is check your age, AMH, and semen report in detail and stop IUI at 3 to 4. Very surprisingly, sometimes doctors suggest doing more than 6 IUI. It is illogical when doctors give false commitment of more than 12 to 18% results in IUI which is next to impossible.
