Infertility: when to meet a Fertility Doctor?

I recommend that you probably don't need to see a  fertility doctor about infertility issues unless you have been trying regularly to conceive for at least one year. However see your doctor, If you are a Woman and: • You are aged 35 to 40 and have been trying to conceive for six months or longer • You are over the age of 40 • You menstruate irregularly or not at all • Your periods are very painful • You've been diagnosed with endometriosis or pelvic inflam-matory disease • You've had multiple miscarriages • You've undergone treatment for cancer  When should a male patient see a doctor? • You have low sperm count or other problems with sperm • You have a history of testicular, prostate or sexual problems • You've undergone treatment for cancer • You have testicles that are small in size or swelling  in the scrotum known as a varicocele • You have others in your family with infertility  problems

PCOS - A Blessing in fertility treatments

As we know – PCOS is POLYCYSTIC OVARIAN SYNDROME which represents with irregular menses, skin manifestation and infertility. India is considered as a hub for PCOS with every zone of India having 12-16% of incidence. Every 6th Infertile couple has PCOS. Therefore it is a question of worry and thus we need to discuss – weather is it really tough or a blessing in fertility treatment to have PCOS in current scenario. As we know that in PCOS, IUI gives 13 -20 % of cumulative pregnancy rate after 6 cycles. But in PCOS ovary is very sensitive to stimulation and IUI stimulation is never controlled properly by any hormone. Apart from results, multiple pregnancy cannot be controlled by IUI Treatment , where ovarian hyper-stimulation is one of the threat behind the treatment. Will the use of Fertility drugs help us in conceiving?

For millions of women struggling to conceive, fertility drugs are the first step in treatment. What do these little pills and injections do exactly? Are they really needed ? How does fertility medication differ? Reasons differ from person to person. For instance, women with polycystic ovarian syndrome (PCOS) generally respond well to clomiphene, but those who don't may be given the insulin-sensitizing drug metformin to help them ovulate. Some women with PCOS respond well to a combination of metformin and clomiphene. Women with hyperprolactinemia have too much of the hormone prolactin in their blood, which interferes with ovulation. Women with this condition who want to conceive will most likely take bromocriptine or cabergoline to restore ovulation. What are the risks of fertility drugs? Is surgical intervention really required before IVF?

It is matter of great debate whether a laparoscopic intervention is really required before starting an IVF treatment. Some clinicians have made it a protocol to conduct laparoscopy in each and every patient which seeks IVF treatment . Following is my honest opinion on surgery before IVF treatment: 1) According to me, Ovarian drilling 'kills fertility'. So it an honest request that please do not proceed with it unless that is the last resort. 2) Fibroids less than 4 cm in size doesn't require surgery before IVF treatment 3) Less than 4 cm endometrioma(chocolate cyst) doesn't require surgery. Only in cases of extreme pain, surgery should be considered. 4) Pre-IVF Hysteroscopy for all it is totally commercialization from some doctors 5) Adeno myomectomy is also unindicated surgery. Why Multiple IUI is not the suggested treatment for infertility ?

Actually IUI is indicated now a days only during following conditions:  Less than 35 years of the age and AMH is more than 2. Polycystic ovary with precise hormone therapy, meaning no over response or no under response for maximum up to 3 to 4 cycles. Donor IUI in 0 count where IVF is not possible or not affordable. Erectile dysfunction Unexplained infertility where duration of infertility is more than 2y and less than 5 with age less than 35 and AMH is more than 2 Male sub-fertility when count is more than 10 mill. And motility more than 10 % and morphology is more than 4% and DNA fragmentation index is less than 50 percent Carry home message here is check your age, AMH, and semen report in detail and stop IUI at 3 to 4. Very surprisingly, sometimes doctors suggest doing more than 6 IUI. It is illogical when doctors give false commitment of more than 12 to 18% results in IUI which is next to impossible.
